Net 30 days with approved credit, F.O.B. Dayton, Ohio. Freight charges will be prepaid and added to the invoice. Purchasers who do not have an account with us may charge their order using VISA or MasterCard or prepay using Company check or money order. To set up an account, please contact our Accounts Receivable Department or fax them your company name, bill to, ship to, buyer name, phone & fax numbers, 3 credit references, name of bank and contact name. C.O.D. orders will be charged a handling fee of $10.00 in addition to applicable shipping charges. All prices and specifications are subject to change without notice. Prices are shown in U.S. dollars and are current at time of printing. Prices charged will be those in effect at the date of shipment, unless advanced quotation has been provided.
We are required by law to add Ohio sales tax to orders shipped within the state. If you are tax exempt, please submit your certificate for our files.
MTM Medical’s goal is to ship the same day as ordered or the next Monday through Friday. Unless otherwise specified, we usually use UPS Ground Service to ship your order. Air shipments, 2nd Day, and Parcel Post are available upon request at extra expense. Any special shipping and handling must be specified when ordering. Unfornately we do not provide international shipping at this time.
Product returns are accepted with prior authorization only and no later than 60 days after the date of shipment. Please contact customer service for a Return Authorization Number(RAN). Be sure to reference RAN on paper work and outside of box sent back. All return merchandise should be sent prepaid unless special arrangements have been made with customer service representative. For merchandise ordered in error there will be a restocking charge of 20% of the invoice value.
Excessive exposure to autoclaving heat may cause warping or decrease structural integrity. Autoclave at 250 degrees F(120 degrees C), under 15 lbs of pressure for maximum of 15 minutes.
